Government Required PrE-Appointment Forms

Medicare Scope of Appointment

The Center for Medicare Services (CMS) requires that we have you compete a Scope of Appointment form so that we are all aware of what Medicare products will be discussed during our upcoming appointment. We must receive this 48 hours prior to the appointment. This must be done prior to every appointment at which you want to consider a new plan or review your current plan benefits.

The Health Insurance Marketplace requires that we obtain written permission to gather and use your personal information to search for plan options, provide you with quotes and if applicable, assist you with your Health Insurance application for coverage and financial assistance. This must only be done the first time you work with us after July 1, 2023.
